🔍 Problem Statement
Electronic Health Records (EHRs) are essential in modern healthcare but often time-consuming, especially when documenting chronic conditions. Doctors must manually type the patient’s history of present illness (HPI), leading to repetitive input, risk of inconsistencies, and decreased face-to-face time with patients.
We identified the need for an AI-assisted, context-aware tool that reduces cognitive load and accelerates chronic care documentation without compromising quality.

🧠 Research & Insights
Through contextual inquiry and stakeholder interviews (physicians, patients, and EHR technicians), we uncovered:
65% of documentation time is spent summarizing chronic conditions.
Doctors often reuse phrasing manually across visits.
Patients expect consistency in how their condition is presented and updated.
💡 Solution
We designed ParaDocs Relay, a lightweight interface integrated within the History of Present Illness (HPI) section of an EHR. Doctors can click the Relay button to trigger contextual suggestions based on the patient’s condition.
